Before your visit with Dr. Zarbalian, please update your active medication list by going to the "My medications" section on the right side of the page (if working on a desktop computer).
For medicare patients, if your insurance has changed, please inform us before your appointment. All patients should also update insurance information under "my account details". If you are a new medicare patient or your info changed, please also upload a photo of your medicare card (and any medicare supplement or medicare advantage cards) in the "Labs and documents" section.
