Before your visit with Dr. Zarbalian, please update your active medication list by going to the "My medications" section on the right side of the page (if working on a desktop computer).
Remove the inactive medications and add new medications.
For medicare patients, if your insurance has changed, please inform us before your appointment. You should also update your insurance information under "my account details". If you are a new medicare patient or your info changed, please also upload a photo of your medicare card (and any medicare supplement or medicare advantage cards) in the "Labs and documents" section.
